Acceptance of Terms
By accessing and using the services of WATERFORD DENTIST PLLC, you accept and agree to be bound by these Terms and Conditions. If you do not agree to these terms, please do not use our services.
Dental Services
We provide professional dental services including but not limited to:
- General dentistry (examinations, cleanings, fillings)
- Cosmetic dentistry (whitening, veneers)
- Restorative dentistry (crowns, bridges, implants)
- Emergency dental care
- Pediatric dentistry
- Orthodontic services
Appointments
Scheduling: Appointments can be scheduled online, by phone, or in person. We will confirm your appointment via phone or email.
Cancellations: We request at least 24 hours notice for cancellations. Repeated no-shows or late cancellations may result in a cancellation fee.
Late Arrivals: If you arrive more than 15 minutes late, we may need to reschedule your appointment to avoid disrupting other patients.
Payment Terms
Payment is due at the time of service unless other arrangements have been made. We accept:
- Cash
- Credit and debit cards (Visa, MasterCard, American Express, Discover)
- Personal checks
- Most dental insurance plans
- Third-party financing options
Insurance
We accept most major dental insurance plans. As a courtesy, we will file claims on your behalf. However, you are responsible for any amounts not covered by insurance, including deductibles, co-pays, and services not covered by your plan. Please bring your insurance card to each visit.
Treatment Consent
Before any treatment, we will explain the proposed procedure, alternatives, risks, and expected outcomes. Your signature on a treatment consent form indicates your understanding and agreement to proceed. You have the right to refuse any treatment.
Patient Responsibilities
As a patient, you agree to:
- Provide accurate and complete health information
- Inform us of any changes to your medical history
- Follow treatment plans and post-care instructions
- Attend scheduled appointments or provide adequate notice for cancellations
- Pay for services in a timely manner
